Revert FileProvider to Keyman app

Starts to address #2039
Move FileProvider back to Keyman manifest and revert authority.
Added Toast notification if 3rd party app doesn't define FileProvider. (info pages fail to properly load)

Store default keyboard/dictionary installation as a preference
...
Per PR comment, use the one-time preference so user has the option to uninstall the default dictionary.
Also made a separate preference for the default keyboard.

Install nrc.en.mtnt.model.kmp as default dictionary
...
* Allow the kmp to be installed if the lexical model doesn't exist
* Modify PackageActivity to silently install kmp files without user confirmation on welcome.htm page
* Fix package ID extracted from `.model.kmp` files to not include `model`.

* Default keyboard doesn't have "custom keyboard" setting so read it as optional parameter
* Don't notify "KEYBOARD_DOWNLOAD_FINISHED" when only downloading lexical model
* Removed registration of lexical model upon download (complications with async task)
